Physical Examination - CHSAA FORM 4 

Students registering for Athletics must have a current physical examination before the student is authorized to participate in practice/tryouts.  To ensure clearance for participation, physicals should be completed and uploaded prior to the first day of practice/tryouts. Completed physicals MUST be uploaded in “Step 4: Physical Forms” of the online registration form.

NON-WINDSOR Student Athletes

Any student not enrolled at WHS and nterested in competing in sports at Windsor HS, must complete a Non-WHS Student-Athlete Information form.  All completed forms by both a parent and School Administrator is emailed to Terri Archer.
